We are looking for an enthusiastic and visionary nurse to join the Transfer of care team at Princess Alexandra Hospital.

You will have a keen interest in complex discharge planning, will have proven communication skills, and be able to work collaboratively with patients, carers and relatives, as well as system partners.

Main duties of the job

The Transfer of care Team (TOCT) has representation from Essex and Herts Social Care teams as well as the Discharge Planning team employed directly by Princess Alexandra.

Collectively the TOCT provide expert discharge planning advice and support for patients across the trust who require support from community services on discharge.

The Transfer of care team are responsible for the management of Head of Transfer of Care, Transfer of Care Matron, Discharge Facilitator, Discharge Support Nurse, Transfer of Care Co-ordinators

The main duties of the role will be to plan, provide and maintain effective Discharge Planning care to patients/clients and their relatives. To continuously develop and enhance knowledge and skills that will directly influence consistent standards of Discharge Planning practice. Working with the ward based and transfer of care team, support the development of student nurses and junior staff, creating an atmosphere conductive to learning and practice development. The role may involve some travel to other hospital sites

Detailed job description and main responsibilities

  • Provide and maintain effective discharge assessment to ensure that patient’s discharges are planned and evaluated in collaboration with the ward multidisciplinary team.
  • Support ward teams to deliver effective discharge planning care to patients/clients admitted to the ward/unit.
  • Seek supervision to ensure that personal clinical practice is continuously developing
  • Participate in appropriate training to ensure safe competent clinical and specialist practice.
  • Ensure that all documentation is comprehensively completed in accordance with the NMC and Trust guidance.
  • Involvement in monitoring and auditing patient care
  • Act at all times in accordance with the NMC Code of Professional Conduct.
  • Participate in the development of operational processes that will ensure timely and appropriate discharge of patients
  • Work towards the development of skills that will contribute to an ethos of nurse led discharge services.
  • Participate in the Trust PRDP process to ensure that you receive and give constructive feedback to enhance personal skill set and the promotion of professional development of nursing within the Trust.
  • Act as supervisor/mentor to junior staff members and student nurse
